Who Is Eligible?

HIVQUAL-US is funded through a cooperative agreement administered by the Health Resources & Services Administration’s HIV/AIDS Bureau.  Ryan White HIV/AIDS Part C and Part D Programs are eligible to participate in the individualized consultation or Regional Groups. 

What Are The Expectations For Individualized Consultation?

• Completion of quality management organizational assessments at baseline
  and 12 months thereafter (Please click here to download the OA)

What Are The Expectations For Regional Groups?

• Actively participate in regional group meetings

• Completion of quality management organizational assessments at baseline
  and annually thereafter

• Submit data as agreed upon by the regional group

• Implementation of at least one QI project per year  

How Do I Get Involved?

Ryan White Part C and Part D programs interested in joining the HIVQUAL-US Program, should download and complete the HIVQUAL-US Referral Form and follow the instructions (second page) for submission.
